LEGAL NOTICE Public Notice – Claystone Healthcare Center This notice is regarding the voluntary withdrawal of the Medicare and Medicaid programs for Claystone Healthcare Center in Ennis, Texas. This notice serves as compliance with 42 CFR 488.456 for Claystone Healthcare Center’s closure effective September 2, 2012. Senior Living Properties, LLC will discontinue all services previously rendered on September 2, 2012 per 42 CFS 489.53(c).
